If you want to become a subcontracted transportation provider for MTM, complete and submit the form on their provider page. Once they receive your information, a team member will contact you.

The Process

Here are the steps you must take to become a provider with MTM:

  1. Provider Application: This tells them more about you and your business.
  2. Interview: An MTM network representative will contact you with more details about the onboarding process and what you can expect.
  3. Contracting: Once MTM is ready to move forward with you, they create and establish an agreement between your two companies.
  4. Credentialing: They will request that you submit all necessary documents to validate you as a certified provider.
  5. Training: An MTM representative will walk you through the trip process and other vital information for onboarding.
  6. First Trip: Once you’ve completed the previous steps, you are ready for your first trip as an MTM transportation provider!
